    The forms listed below must be completed and mailed to the attention of the Office of Student Success & Retention Services at Bethune-Cookman University using the address below:

    These forms must be completed by all international students in order to have an I-20 for their acceptance at Bethune-Cookman University.

    You are required to provide a notarized affidavit of financial support. The affidavit must be from the individual that will be providing the bank statement and the actual funding of your educational expenses while attending Bethune-Cookman University.
